12/7/2023 EnchantaVaughn Approved Upon arrival to the home setting, QMHPA opened up session with Ms. Broadnax and asked about any mental health emergencies at this time. Ms. Broadnax did not present any symptoms of SI or HI. QMHPA informed Ms. Broadnax that the purpose of this activity was to demonstrate appropriate calming techniques when experiencing depression through role play. QMHPA explained to Ms. Broadnax about being able to endure in enjoyable activities, group sessions and hobbies. QMHPA explained to Ms. Broadnax the difference from relaxation and just lying in the bed when she is bored. QMHPA encouraged Ms. Broadnax to participate in more community activities to increase her social skills. QMHPA discussed with Ms. Broadnax presenting positive behaviors throughout the community by communicating with people through effective communication skills. QMHPA explained to Ms. Broadnax how isolative behaviors can cause self-destructing behaviors. QMHPA identified with Ms. Broadnax inappropriate ways to manage depression. Ms. Broadnax appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Broadnax was able to perform some of the exercises that was demonstrated. During the skills building activity, Ms. Broadnax verbalized to QMHPA that she will have an attitude with others and she doesn’t like to be bothered by others when she is depressed. Ms. Broadnax verbalized to QMHPA during the session that she enjoys walking. Ms. Broadnax stated she does relax but has things on her mind or just lay in the bed because she is tired. Ms. Broadnax discussed with QMHPA that she felt as though the participating out in the community would assist her with being able to increase her social skills a little more because she was out and around others and she had no other choice but to talk to them. She indicated to QMHPA at times, she wish she didn’t have a mental issue because she doesn’t like taking medications and she doesn’t like being depressed or moody all the time.
